How To Apply?

Before applying you must contact this number: 772-646-5350 for a level two background check  between the hours of 9 AM-2 PM Monday – Thursday.

After you pass your level two background check, you are to go to your local Sheriff’s office in your county and get a local background check.

When the background check is ready, you can submit it one of two ways:

OPTIONS:

A) Give this email to the Sheriff’s office: info@trinityomniscientcare.com, or

B) Return to this portal and upload a CERTIFIED COPY of your local background check along with the document in Step

  1. Fill out and sign the Good Moral Attestation.

IMPORTANT: The Good Moral Attestation has to be signed and uploaded to this portal within 30 days of receiving the local background check: NO EXCEPTIONS.

    1. Fill out the application form,
    2. Upload all of your documents once you have completed the application.
      • Driver’s License,
      • Social Security Card,
      • Car Insurance,
      • Car Registration,
      • Resume,
      • High School or College Diploma/Degree.

IMPORTANT: After uploading all of your documents, you must begin training. You must complete the zero tolerance training before you start work.

Once you have completed all your uploads and courses, you will be sent a link to plug into the iConnect/EVV to log in for employment.

The rest of the training classes must be completed within 10 days of beginning employment.
